Volunteer Leader Trainers are needed for a Chronic Disease Self-Management Program. This self-management program is evidence-based and uses transformative techniques to dramatically improve health outcomes for older adults.
As a Volunteer Leader you will:
- Complete a four day training to become certified in a Chronic Disease Self-Management Program.
- Attend an additional day of training for the Building Better Caregivers Program.
- Co-facilitate small groups with another leader online and in person.
- Lead at least one workshop within one year.
- Change lives.
Chronic Disease Self- Management Program Training Dates: December 7, 8, 14 & 15, 2021 9:30 am - 4:30 pm
Crossover Training: Building Better Caregivers: December 16, 2021 9:30 am - 4:30 pm
